FINANCIAL POLICY (Our Comfortable Waiting Room) To provide our patients with excellent quality service, we maintain the following financial policies: • Payment or payment arrangements are required at the time of service. • Payment forms accepted: cash, check, Mastercard, Visa, American Express & Discover • We offer payment plans through Care Credit Services (see below for information). This convenient service is a very popular payment plan option with our patients. • New patient emergencies must be paid in full at the time of service. • Patients will be charged $40 for missed appointments (without 48 hours advance notice). • Patients are responsible for balances > 60 days (if insurance has not paid). • A service charge will be assessed for all returned checks. • Patients must provide the office with accurate insurance billing information at the time of service, or they will be responsible for payment in full.
